Care Manager MSW - USFHP

Coordinates care for Pacific Medical Centers' Managed Care patient in order to assure cost effective, appropriate utilization of health care resource. Case Manager manages, directs, and coordinates the entire spectrum of care for patient throughout a variety of settings, both inpatient and outpatient. This position assists physicians (internal and external) with management and coordination of care. Works at varied clinic and provider locations as needed.

Location: Hybrid. Three days onsite in Renton

Schedule: M-F, 8:00 am – 5:00 pm

Required Qualifications:

• Master's Degree Social Work

• Within 18 months of hire: Certified Case Manager

• Upon hire: Specific Training in Mental Health First Aid/Suicide Prevention or equivalent - current and maintained ongoing

• 5 years Clinical experience.

• 2 years In a Behavioral Health setting.

• Utilization review/case management.
