The Behavioral Health Case Manager (BHC) provides case management, care coordination services and short-term therapeutic interventions for at-risk geriatric patients, including patients with complex medical, behavioral and psychosocial issues. The goals for this position are to ensure a systematic approach to the coordination of health services for patients along the care continuum in order to improve the patient experience, contain health expenditures and improve quality. BHCs assist members with coordinating access to appropriate care and services in an effort to improve or maintain the psychosocial, functional status and independence.

Required Qualifications:
Master's Degree in Social Work from a CSWE accredited school of social work or
Upon hire: Washington Clinical Independent Social Worker License
1 year Clinical experience in behavioral health related field
Preferred Qualifications:
1 year Managed care experience and/or care management experience
Direct Experience in providing Behavioral Health Care Management

About the Team
Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

Pay Range: $34.66 - $54.87
The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
