We are seeking a Supervisor Social Services - Care Managemen who will coordinate Benedictine's social services program providing high-quality care and services reflective of professional standards of practice and in compliance with all federal, state and local laws. Responsibilities include supervising, staffing, budgeting for and evaluating the program, working as part of the interdisciplinary team, and ensuring appropriate and adequate services are provided. Ensures that social services staff are knowledgeable about resident's rights in accordance with federal, state and local laws. Serves as a resource person to families regarding available programs and services, and identifies special concerns through use of family conferences and informal visits.

In this position you will have the following responsibilities:

  • Establishes and maintains strong ongoing relationships with referring agencies, hospitals, and individuals in the community. Coordinates periodic visits to neighboring hospitals in an effort to maintain good relations. Sponsors visits by referring social services workers to Benedictine.

  • Manages the daily operations and staff of the Social Services Department including: hiring, training, scheduling, coaching, evaluating performance, disciplinary action and discharges.

  • Plans and manages the department's expenses and budget.

  • Responds to admission inquiries and completes admission forms. Ensures that necessary authorizations, screening and funding are in place prior to the admission of any resident. Coordinates admission with DNS, Administrator, and other disciplines when appropriate.

  • Collects social history data and completes assessments on residents upon admission and as needed according to department guidelines.

  • Participates in interdisciplinary team care conferences by providing placement goals and objectives for residents, and by acting as a resource person to help coordinate and establish a resident care plan. Works closely with Resident Care Managers, Chaplains, Life Enrichment Coordinator and Charge Nurse in identifying and meeting on-going social and emotional needs of residents.

  • Develops a therapeutic relationship with families/guardians, facilitating communication with the interdisciplinary team. Communicates issues and follow-up to the appropriate person(s) and documents information in the medical record. Assists other professional staff by communicating special needs and concerns to families, and conversely by bringing concerns of residents and families to staff. Serves as a resident advocate, especially in relation to the 'Resident Bill of Rights.'

  • Assesses a resident's potential for discharge and the availability of alternate living services no less than quarterly. Actively participates in the development of discharge plans, in coordination with the interdisciplinary team, families and outside agencies.

  • Coordinates financial and other resources to meet needs regarding ICF/SNF stay. Updates Utilization Review Case Manager on patients receiving skilled services as necessary.

  • Participates in Ethics, Palliative Care, Transitions of Care, Quality Assurance and other committees as appropriate. Actively participate in Leadership Team meetings and mandatory training sessions.

  • Plans and directs department staff meetings and training sessions as needed.

Qualifications:

Required qualifications for this position includes:

  • Bachelor's Degree in Behavioral Science or related field.

  • 2 years of work experience in long term care facility or related field that includes:

  • 1 year of experience supervising staff.

  • In Compliance with the Department of Human Services (DHS), a criminal background check is required for this position.
