Nurse Practitioner or Physician Assistant Home Visit

Providence Medical Associates is seeking an experienced Nurse Practitioner or Physician Assistant to join our team in Los Angeles, California. Be part of an innovative group that is passionate about serving our diverse patient population. You will visit high risk seniors in their homes and work collaboratively with physicians, care managers and other healthcare professionals to help improve the quality of their lives and reduce hospital admissions. Experience working in a collegial and highly supportive environment while living in a beautiful Mediterranean climate.

• Outpatient care only

• Full time position, Monday to Friday

• Applicants must have a minimum of 3 years’ experience managing senior patients with conditions such as heart failure, diabetes, chronic kidney disease, emphysema, dementia.

• Board certification as a Geriatric NP or Adult-Gerontology NP preferred but not required

• Eligible for Public Student Loan Forgiveness (PSLF)

• Compensation is between $175,000 and $230,000 per year

Where You’ll Work

Providence Medical Associates is a 200-provider multispecialty medical group in Los Angeles serving communities in the greater South Bay, mid-Los Angeles, and San Fernando Valley. PMA provides the full spectrum of integrated care with a strong base of adult primary care physicians, pediatricians and OBGYNs, a broad range of adult specialties, four urgent care centers, a hospitalist team, post-acute ambulatory and SNF care, and pharmacist and behavioral health support.
