Providence Health & Services Registered Nurse (RN) Palliative Care Services in Everett, Washington
Providence is calling a Palliative Care Registered Nurse (RN) (1.0 FTE, variable) to work in the Providence Medical Group (PMG) Inpatient Palliative Care Consult Team at Providence Regional Medical Center (PRME) in Everett, Washington.
This position will encompass a dual role providing RN palliative care services in the Emergency Department and triage consult services for the inpatient Palliative Care Consult Team with variable work hours.
The Palliative Care RN will be responsible for working closely with Emergency Department physicians and caregivers to facilitate palliative care interventions and goals of care conversations. This caregiver will work in collaboration with the inpatient Palliative Care Team, yet work independently in the Emergency Department, providing direct evidence-based palliative care clinical and educational support. This position will act as a liaison between the Emergency Department and Inpatient Palliative Care Team to ensure access to early, concurrent, and evidence-based palliative care services for patients, colleagues, and the community.
Inpatient Consult Team :
The Palliative Care RN will act as a liaison between the Inpatient Palliative Care Team and referring providers. This caregiver will work with an interdisciplinary team comprised of physicians, nurse practitioners, social workers, and chaplains to coordinate patient care. The Palliative Care RN will review patient charts, employ the consult triage process to assess and prioritize patient needs, and act as an ongoing communication liaison between the team, referring providers, care management, and hospice, to ensure completion of patient care processes.
In this position, the Palliative Care RN will:
Facilitate conversations with patients and families to develop goals of care including designation of surrogate decision makers, Physician Orders for Life Sustaining Treatment (POLST) forms, and assess patient/family understanding of illness and treatment options;
Be required to demonstrate impeccable communication skills;
Demonstrate strong technical nursing and Palliative Care expertise;
Demonstrate a strong understanding of Palliative Care services and support;
Act a liaison between the inpatient Palliative Care Team and referring departments and physician groups;
Collaborate with the inpatient interdisciplinary team to develop goals, objectives, and action plans;
Identify ethical/moral conflicts and refer to the Ethics Committee for assistance as needed;
Assist in the development of evidence-based nursing practices, procedures, and guidelines;
Assist in all facets of care coordination for palliative care referrals;
Develop, plan, and coordinate patient, staff, and community education regarding palliative care issues; and
Participate in interdisciplinary meetings as needed for complex care management.
Bachelor’s Degree in Nursing.
2 years clinical experience in a hospital acute care setting with a background in critical care, oncology, palliative care, or hospice nursing.
Actively work to complete Hospice or Palliative Care Nursing Certification within 2 years of hire.
Current Washington State Licensure as a Registered Nurse.
Master’s Degree in Nursing or similar field.
Experience in Palliative Care inpatient or outpatient settings.
ELNEC Core Training, CAPC or Hospice Education Network core curriculum.
